We are looking for compassionate caregivers to provide assistance in daily living and personal care services in patients' homes in accordance with an established care plan, and under the supervision of a RN Case Manager.

Responsibilities :
- Follow client care plans
- Establish and maintain effective and professional communication and a strong relationship with clients, their family members, and co-workers
- Provide personalized in-home support to senior clients
- Help clients with all personal care needs
- Help keep clients' environment clean and organized
- Remind client to take self-administered medications
Required Skills, Education and Certifications :
- CPR Certified
- Recent negative TB test results (PPD or Xray)
- Previous experience as a caregiver, a plus but not mandatory
- Excellent customer service and supervisory skills
- Computer proficiency and ability to document timely and accurately notes in system
- Polished communication and presentation skills and ability to develop excellent relationships
- Current driver’s license and proof of insurance
