COMPANY INFORMATION

Rockford Township General Assistance is a local government agency that is required by the State of Illinois to provide General Assistance welfare to qualified individuals. General Assistance is provided to eligible individuals living in Rockford Township with no minor children in the household and little to no income.

JOB DESCRIPTION / SUMMARY

Under the direct supervision of the Rockford Township Supervisor, employee is responsible for case management services to a diverse / high risk adult population. The position will include direct and indirect client service for General Assistance and Emergency Assistance. The Case Manager will learn the General Assistance Handbook and be able to apply guidelines to each applicant’s case. The Case Manager will determine applicants’ eligibility for the program by conducting home visits and monthly client interviews.

JOB TASKS & RESPONSIBILITIES

  • Responsible for maintaining a valid driver’s license and proof of auto insurance; must have personal vehicle available for home visits
  • Conduct intake interviews for applicants seeking General Assistance and Emergency Assistance
  • Responsible for conducting home visits for General Assistance / Emergency Assistance / Senior Assistance applicants following the minimum City of Rockford building code requirements to complete the eligibility process
  • Conduct intensive case management following the General Assistance Handbook Guidelines and policies and procedures set forth by the Rockford Township Supervisor for General Assistance recipients
  • Maintain monthly reports and statistics on client data
  • Maintain monthly calendar for client appointments
  • Maintain client files and records that are clear, concise and accurate
  • Adhere to agency policy, procedures and the professional code of ethics

EDUCATION & EXPERIENCE

  • Bachelor’s Degree in Social Work or related field or 3+ years of relevant experience required
  • Previous Case Management experience preferred
  • KEY COMPETENCIES

  • Proficient in Microsoft Office including Outlook, Word and Excel
  • Ability to build and maintain professional connections with other social service agencies in the area
  • Knowledge of community resources and counseling / social work practices with high-risk populations
  • Excellent written and verbal communication skills, ability to establish rapport
  • Ability to work independently and in a team
