NURSE CASE MANAGER-OBAT RN HOSPITAL AMBASSADOR
Lynn Community Health Center is a Federally Qualified Community Health Center that provides addiction services for all patients who use substances with a no wrong door, harm reduction approach to care.
We are committed to meeting patients' needs whether they are seeking recovery or continue to use. We provide primary care and addiction services within the Office Based Addiction Treatment (OBAT) model of care on various integrated primary care teams at the health center.
We also offer services to patients referred from other settings for stabilization and support.

In this role you will :
Rounding with the ACT team
Meeting with the BRIDGE CLINIC Team
Coordinating with inpatient social work and case mgmt to enhance discharge planning
Meeting with admitted LCHC patients (established or intended) to build engagement in care
Representing LCHC addiction services at meetings of hospital care providers and administrators
4 hours per week devoted to population health mgmt related to reviewing lists of currently or recently admitted patients, identifying outreach opportunities, etc
The successful applicant will offer :
- Graduate of an accredited nursing program.
- RN with current Mass. Licensure
- Case Management experience preferred.
- Bilingual in Spanish / Khmer / Russian beneficial
- A desire to work as a collaborative member of a multi-disciplinary team.
