Meets safety needs of patient and using equipment safely and properly (foot stools, side rails, O2)Gives personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assignedAssists in dressing and undressing patients as assignedPlans and prepares nutritious meals, including shopping, as assignedAssists in feeding the patient as assignedTakes and records oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed / demonstrated skills competency and when on the plan of careProvides proper care and observation of patient's skin to prevent breakdown of tissue over bony prominenceReports on patient's condition and significant changes to the Case ManagerAlso aware of the caregiver or other individuals living with the patient and interpersonal issuesAssists in ambulation and exercise as instructed by the hospice nurse or therapistProvides normal range of motion and transfers / positioning as directedAssists with self-administered medications as allowed by state regulations, such as remindersOffers and assists with bedpans and urinalsProvides assistance as assigned with light laundry needsPerforms range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed / demonstrated skills competencyProvides respite for patient's / family / caregiver when on-site, as appropriateKeeps patient's living area clean and orderly, as assignedAdheres to the organization's documentation and care procedures and standards of personal and professional conductParticipates in quality assessment performance improvement teams and activitiesRespectful of patient and family / caregiver environment and patient's personal needsQualificationsMeets the training requirements of the StateCompleted a minimum of seventy five hours of classroom and supervised practical training Sixteen hours of classroom must be completed prior to a mandatory sixteen hours of practical trainingMinimum of six months experience including providing personal careAt least eighteen years of ageAbility to read and follow written instructions and document care givenUnderstands hospice philosophy, comfortable providing specialized care to the terminally illSatisfactory references from previous employersAbility to work with little direct supervisionSecure with issues of death / dyingProvides a calm manner when in a patient’s homePossess and maintains current CPR certificationDemonstrates tact, patience, and good personal hygieneSuccessfully completes a probationary periodThis probationary period is to ensure the aide is competent in the above qualifications and is documented by the supervisor and the employeeStrong oral and written communication skillsGood organizational skillsAbout our Line of BusinessAt Embrace Hospice & Home Health, our primary ambition is to bring first-class care to the warmest and most serene environment possible : home.
