JOB SUMMARY

AtHouston Methodist the RN Wound Ostomy Continence position providesexpert integrated costeffective quality patient care through thecoordination of clinical care for highrisk patients with complexissues across the continuum of care settings serving as theclinical resource and consultant for acute and rehabilitative careof patients with selective disorders of the gastrointestinalgenitourinary and integumentary systems.

Responsibilities for thisposition include but are not limited to draining wounds ostomiesfistulas dermal ulcers tubes and continence disorders.

The RN WoundOstomy Continence position provides oversight to the woundtreatment plans and participates in performance improvement reviewactivities.

This position maintains working knowledge of thehospital policies and procedures as well as regulatory guidelines.As a nurse clinician the RN Wound Ostomy Continence positionintegrates knowledge of advanced pathophysiology human responses tohealth problems and nondisease based etiologies (selfcare behaviorsstress coping behaviors environmental and social demands / resources) to assess plan and coordinate care and monitor andevaluate patient outcomes.

This position requires the ability toassess data reflective of the patients status and appropriatelyinterpret information relative to the patients agespecific needs.

The RN Wound Ostomy Continence position is an active participant inthe collaborative practice framework.

Utilizesexpertise and knowledge of skin integrity wound ostomy andcontinence care to develop and update standards of care.

Implementsnursing actions to maintain appropriate clinicaloutcomes.

  • Documents in the electronic medicalrecord as appropriate for their specialty and reviewsinterprofessional documentation to ensure / validate accuracy.(EF)
  • Responsible for the oversight andconduction of the Pressure Injury Prevalence surveillance inconjunction with Quality and Patient Safety and the development ofresulting action plans. (EF)

  • Develops proposals forimplementation of new programs projects and / or nursing research.Fosters identification and implementation of innovative solutionsto improve patient care or unit operations such as reduction ofhospital readmissions or other departmentspecific measures byleading and / or participating in hospital projects and sharedgovernance activities.   • Leads andidentifies opportunities for practice changes and performanceimprovement. Investigates and incorporates evidencebased practiceswith input from shared governance and leadership when appropriate.

Supports change initiatives and adapts to unexpected changes.(EF)

Expands individual nursing knowledgecompleting and updating the individual development plan (IDP) on anongoing basis. Coaches staff to grow in knowledge abilities skillsand attitudes.

EDUCATIONREQUIREMENTS

  • Bachelorsdegree from an accredited school of Nursing
  • Masters degreepreferred

EXPERIENCEREQUIREMENTS

Two years ofclinical nursing experience in specialty or relatedarea

CERTIFICATESLICENSES AND REGISTRATIONSREQUIRED

  • Texas RN licenseor temporary TX RN license should obtain permanent license within90 days. Compact license acceptable according to current Board ofNursing requirements
  • American Heart AssociationBasic Life Support (BLS)
  • American NursingCredentialing Center (ANCC)recognized Wound and Ostomycertification or related area within six months ofeligibility
