About The Position
Job Title : Rehabilitation Therapy Assistant Job Specialty : Physical Therapy Assistant Job Duration : 13 weeks Shift : Monday to Friday Guaranteed Hours : 32 hours per week Experience : New graduates are welcome, Skilled Nursing Facility (SNF) experience preferred License : State license required Certifications : COVID test required 72 hours prior to start date, with results issued 48 hours before starting Must-Have : - Resume - License verification - Three references Job Description : - Ensure prescreen is filled out and attached upon candidate submission - Complete county criminal check for the past 7 years for all counties lived and worked - Complete federal criminal check for 7 years or more - Conduct multi-state criminal search for 7 years or more - Perform nationwide sex offender search for 7 years or more - Conduct HHS / OIG search - Perform GSA search - Conduct social security number trace - Provide 2 Step PPD or Quantiferon test results, or proof of positive PPD with a chest x-ray - Conduct SAM search - Complete Oregon state background check (facility run) Please make sure all required documents are submitted to avoid candidate declination.
