Description

The Senior Community Health Worker is responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.

Required Qualifications :

  • Education to meet certification, license or registration requirement.
  • Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving.

Caregivers will be expected to provide proof of driver license and auto insurance upon request.

1 year - Experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience, or Bachelors degree in lieu of experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby / on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
