Job Title : Hospice RN Case Manager
Duties & Responsibilities : Hospice RN Case Manager sought for 13 week assignment, with the potential to extend. Schedule would be 5x8 days, Monday-Friday, with no on call or weekends required.
Mileage is reimbursed at $0.50 per mile.
Minimum Requirements :
- SC or Compact RN license
- 1+ year hospice in home settings experience within the past 3 years
- Must have experience with case managing
- HCHB experience required
