Qualifications :
- NCLEX-RN certified nurses with 3-5 years of recent Telephone Triage experience / Call Center Experience
- Licensure in at least one US state is required. Nurses registered in the state of Illinois are highly preferred
- Preferred candidate with minimum 2 years of clinical experience in a physician office, one or more specialties, home health, critical care and / or emergency room
- Demonstrates empathy and compassion while interacting with patients, understanding their concerns, and providing support.
- Clinical Assessment : Skilled in handling different types of calls from patients / care givers, assessing patient symptoms and determining whether medical attention is necessary.
Ability to provide guidance on self-care when appropriate.
- Experience working with Electronic Medical / Health Records and documenting electronically.
- Experience with various computer software, knowledge of Microsoft Word, Excel, Outlook, and the internet
- Typing skills to prepare forms and correspondence with speed and accuracy.
- Skilled in speaking English clearly and distinctly, with appropriate grammar. IELTS / TOEFL certification would be a value add
- Demonstrated ability to work as part of a team while functioning independently, maintaining confidentiality in all assignments, and showing initiative in identifying and solving problems as they occur.
Role and Responsibility :
- As a Telephone Triage Nurse accept telephone calls from patients with varying levels of acuity and types of medical concerns including but not limited to
- Performs to the level of a licensed registered nurse as dictated by the Illinois Nurse Practice Act
- General health inquiries
- Chronic disease management
- Medication Management
- Mental health concerns
- Promote clear and coherent verbal communication following an appropriate line of questioning & clinical assessment
- Medication advice : Provide advice on over-the-counter medications and recommend appropriate dosages
- Referral to Healthcare providers : Direct patients to the appropriate healthcare provider if necessary, such as primary care physician, specialist or emergency department
- Follow-up care : Provide follow-up care, checking in with patients to see how they are doing and ensuring that they are following the recommended treatment plan
- Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards;
making or recommending necessary adjustments
- Manage prescription refills per protocol.
- Accurately documents symptoms / complaints, nursing assessments, advice provided and patient / caller response in patient and department records.
- Maintains accurate and detailed patient records and pertinent information; posts tests and documents patient education provided.
Enters all pertinent communications with the patient into the Medical Record
- Resolves patient problems and needs by utilizing multidisciplinary team strategies as appropriate and assists in the development of case management metrics to identify opportunities to improve care
- As a member of a health care team, functions in the role of patient advocate by being in the position of initial contact for patient-related issues
- Follows patient privacy in accordance with HIPAA.
- Ability to make sound decisions quickly and effectively, especially in urgent or critical situations
- Conducting outbound calls to patients in need of medical advice or follow-up care
