• Vancomycin, aminoglycoside, warfarin, heparin dosing· Medication order verifications· IV product verifications· Renal Dose adjustments, IV to PO ConversionsREQUIRED CERTIFICATIONS : · Texas Pharmacist License· IV CertificatePreferred or nice to have : candidates with these skills will be considered first.
• Critical Care Experience· NICU Experience· Pharmacy Residency· Board CertificationList typical procedures performed on unit(s) : · Medication Order Verification· Medication Dosing Consults (Vancomycin, Aminoglycosides, Warfarin, enoxaparin, acetylcysteine)· Antimicrobial stewardship, opioid stewardship· Working through automatic protocols such as IV to PO conversations, Renal DosingBest personality Fit : · Team player· Positive attitude· Collaborative
