Exceptional Home Care has serviced LVN shift care for sub-acute adult, and pediatric patients for over 20 years.

We are Seeking Part time Registered Nurses, who have great communication skills, proficient computer, and documentation skills, can turn in their work on time, and who are reliable.

QUALIFICATIONS :

• 2 years or more with an RN License
• 2 Years of Home Health experience.
• 2 year of Case Management preferred.
• Experience with G-tube, Trach, and Vents.
• Bachelors Degree (preferred)

CREDENTIALS :

• Current RN License, CPR, TB or Chest X-Ray, Physical, Auto Insurance, Driver License, Social Security Card, Covid Vaccine Card
• Or Declination Statement of Covid Vaccine)
• Current Trach / Vent Certificate *(Valid within the last 3 yrs) If nurses do not have a current Trach / Vent Certificate
• our agency will refer you to a 3rd party to get your certificate within 30 days of hire date.

ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES :

• Some computer, tablet, or smart phone exp. preferred. Electronic medical record charting is done for daily documentation for the RN Supervisory Form, Adult and Pediatric Recertification Assessments, Medication Profile, Physician Orders, Clock in / Clock Out.
• Some knowledge of EMR (Electronic Medical Record) charting. We are currently using Synergy for our EMR.
• Report and document any changes in regards to the patients medical condition, medications, feedings, etc.. to patient's medical team either in person, via zoom, phone call, etc.

and relay information to agency's LVN's, Director of Nursing, and office staff as needed to ensure continuity of care.

Admit new patient case referrals. Reviews available patient information related to case. Instructs and educates new patient parents / guardians on the policies and procedures of the agency upon admission.

Goes over consents to treat and authorization of service forms.

• Performs Initial Home Safety walk through and Head to Toe assessment.
• RN Supervisory Visit due every month, need to update and complete Medication Profile, Physician Orders.
• Recertification Visit due every 60 days - Update and complete Case Conference, Medication Profile, Recertification assessment for both pediatric and adult patients and Physician Orders.
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Assists in the supervision of field clinicians while out on duty. Counsels and educates field clinicians on an as needed basis.
• Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
• Complies and follows agencies policies, procedures, guidelines, and professional standards

Our agency gives the parent / guardian the opportunity to have time in their day to run errands, take care of personal and medical appointment, spend much needed family time or even to have time for self-care.

Exceptional Home Care makes it a priority to find a nurse that the family can trust to leave their loved one in our hands.

We book schedules with patients one month at a time. If you’re an LVN and are open to work open Full Time, Part Time or Per Diem we have cases that are available.

We are looking for reliable LVNs who show up to work, have minimal cancellations, and perform their LVN duties proficiently and effectively.

Our RN Supervisors schedule / book monthly in house visits in order to get a report on any changes that have happened within the past 30 days and supervise our field LVN's to ensure patient is receiving proper nursing care.

Benefits after 90 day probationary period :

Pay Days are every 10th and 25th of the month.

Direct Deposit. Nurse must bring a Direct Deposit form and it will take 2 pay periods for it to kick in.

Full-time Employees benefits : (Those who work 40 hrs. a week or more are considered full-time)

Sick time 40 hours per calendar year (5 8 hour shifts)

401K

Medical Insurance Kaiser, and AFLAC.

We offer Referral Bonuses to nurses that refer to an LVN’s and RN’s. Bonuses are issued once the new hire completes 400 hours worked.
