The Advanced Practice Provider exercises independent judgement in the assessment, diagnosis and initiation of medical processes and procedures, in accordance with the licensure regulations of the Commonwealth of Virginia, and as delineated in the written Scope of Practice and Protocol.

Demonstrates respect for and an understanding of other clinical disciplines while using an integrated team-based approach to achieve desired patient outcomes.

Utilizes evidenced based standards, guidelines and protocols for care delivery. Participates in quality and best practice initiatives by utilizing standards and guidelines for cost effective care delivery to continuously improve patient care and enhance outcomes.

Performs high-quality clinical practices and effective patient management under physician supervision by following state regulations, including primary and secondary specialty care in clinical and surgical practice settings.

Job Responsibilities

Inova Medical House Calls is looking for a Nurse Practitioner to support our communities in Northern Virginia. The Medical House Calls program brings primary care into the homes of adults over the age of 65 who cannot drive and reside within the program’s extensive 52 zip code coverage area.

Program Overview

The Medical House Calls program is specifically tailored to support elderly patients in their desire to age comfortably at home, aiming to minimize hospital readmissions and overall healthcare costs.

This initiative is driven by an interdisciplinary team that delivers comprehensive primary care directly to patients in their homes or assisted living facilities.

Typically serving patients aged 65 and above who face challenges in attending traditional medical appointments, the program primarily assists individuals managing multiple chronic conditions, coping with changes in functional ability, handling polypharmacy, and dealing with frailty or debility.

The primary care services provided through this program involve intensive patient management and meticulous care coordination, with a particular focus on advance care planning.

To ensure optimal patient outcomes, the program team establishes robust partnerships with various healthcare entities, including inpatient teams, hospice agencies, skilled nursing facilities, physical therapy, occupational therapy, mental health services, and county support services.

The geographical coverage of Medical House Calls extends across approximately 48 zip codes in Northern Virginia.

Job Description

Provides safe and effective care to ambulatory patients under the supervision of a collaborating physician. Utilizes independent judgment in the assessment, diagnosis, treatment planning, and provision of ongoing care to ambulatory patients that is in accordance with nurse practitioner licensure regulations for the Commonwealth of Virginia and delineated in the written Scope of Practice / Protocol for Nurse Practitioner.

Experience

Minimum 3 year as Nurse Practitioner

Licensure, Education, and Certifications

  • State License
  • Valid DEA
  • Graduate from an accredited program
  • Practices in accordance with safe and effective standards of care and ensures compliance with professional and ethical standards of practice
  • Communicates appropriately with physicians, peers, nursing staff and other healthcare providers
  • Develops differential diagnosis and plans of care based on data collection and revises based on physiologic and psychosocial findings, interprets and synthesizes laboratory, imaging and other pertinent data, and reviews patient plan of care with interdisciplinary team and with the patient and family
  • Evaluates and documents patients' response to care, and evaluates impact of illness on patients' health status, functional status, nutritional status, sleep and rest patterns, family and social relationships, and establishes a health promotion strategy
  • Assumes clinical management for the acute care patient population in collaboration with physicians in practice
  • Provides accurate record keeping as evidenced by complete and pertinent history and physicals, pre-operative / progress notes, discharge summaries and data information sheets
  • Serves as a role model and preceptor for new Advanced Practice Providers and students
  • Participates in and supports the departmental, hospital and system-wide quality performance improvement initiatives
  • Supports departmental cost savings strategies and provides care in an efficient and cost conscious manner
  • Assists in the development and revision of standards for patient populations and integrates evidence-based research findings into clinical practice
  • Participates in clinical research and data collection and assists in projects and planning for continual development of respective specialty
  • Provides clinical and professional educational programs to improve staff knowledge
  • Proactively assists peers with work tasks and takes initiative to complete work beyond daily assignments
  • Works aggressively to ensure patient satisfaction
  • May perform additional duties as assigned

  • Certification - Basic Life Support upon start; Advanced Cardiovascular Life Support (for Critical Care) upon start; Pediatric Advanced Life Support (for Pediatrics) upon startLicensure - Licensed Physician Assistant or Nurse Practitioner;
  • If Nurse Practitioner : RN Compact License(depending on State of primary residence) AND a license (or eligibility for) as a nurse practitioner or physician assistant in the Commonwealth of Virginia AND a "Practice Agreement" signed by sponsoring / supervising physician(s), AND a written "Scope of Practice / Protocol" signed by the supervising physician, AND "Drug Enforcement Administration License" (if prescribing controlled substances)Experience - No prior experience required;
  • New graduate consideredEducation - For Nurse Practitioners : Bachelor's Degree in Nursing and Master's Degree in Advanced Clinical Practice or graduate of a post-masters certificate program for nurse practitioners, OR certificate from an education program that is accredited by the Council of Nurse Anesthesia Educational Programs / Schools, American College of Nurse Midwives, American Association of Colleges of Nursing of the National League for Nursing;
