We are seeking a dedicated Registered Nurse to join our team in Des Moines, IA, for a part-time remote position, offering flexible hours of 10-12 hours a week.
This role involves monitoring and assessing patient health through Remote Patient Monitoring (RPM) and Remote Therapeutic Monitoring (RTM), identifying abnormal readings, and intervening as needed.

Key Responsibilities :
Monitor and assess patient health through RPM and RTM, identifying abnormal readings and intervening as needed.
Provide personalized coaching and education to patients with chronic conditions, supporting adherence to treatment plans and lifestyle changes.
Collaborate with Nurse Practitioners and other healthcare providers to ensure comprehensive, coordinated care and escalate patient needs when necessary.
Maintain accurate and timely clinical documentation, ensuring compliance with healthcare regulations and privacy standards.
Qualifications :
Experience : Previous experience in a similar role preferred.
Certifications : Active RN license in Iowa.
Skills : Strong communication and interpersonal skills; proficiency in EMR systems.
EMR Proficiency : Experience with AthenaHealth EMR system is a plus.
