About The Position
- Job Title : RN Case Manager - Home Health Job Specialty : Home Health Nursing Job Duration : 13 weeks Shift : Day - 5x8 - 8 : 00 AM - 4 : 00 PM, with every 4th weekend rotation Guaranteed Hours : 40 hours per week Experience : Minimum of one year of Medical / Surgical nursing experience required;
- home care / hospice experience preferred License : Current license to practice as a Registered Nurse Certifications : Current American Heart Association BLS certification Must-Have : Graduate of an approved, accredited school of professional nursing;
BSN preferred- Demonstrated excellent verbal and written communication skills- Strong interpersonal skills- Demonstrated organizational and time management skills- Current driver's license, a good driving record, and reliable transportation Job Description : Provide skilled care and case management to patients and families- Operate in accordance with the State Nurse Practice Act and accepted professional standards- Utilize nursing theory and processes for services- Communicate and document observations and assessments- Promote and maintain health through direct care, teaching, counseling, and rehabilitative measures- Report to the Clinical Team Manager- Follow established organizational and departmental policies and procedures
