Salary :
The Foster Care Case Manager / Adoption Specialist will work with the child and foster parent(s) to help the child reach their goals. This role will also work with foster families to ensure their growth and development. In addition, this role will equip the child and foster parent(s) with the support and resources needed to achieve permanency. Case Managers will work with clients in an office, community, school setting, and in the foster home.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
PLACEMENT / INTAKE / ASSESSMENT
- Participate in the referral matching process for children and families, while advocating for best possible home match.
- Facilitate and develop each childs Initial Service Plan
- Facilitate and / or ensure completion of appropriate Assessments, i.e. Biopsychosocial, CANS, Independent Living, Diagnostic, etc.
- Refer foster child to applicable support services, i.e. behavioral health, independent living, medical, educational, etc.
- Ensure initial legal documentation is obtained prior to placement or service delivery
- Ensure that appropriate safety plans are developed during intake
- Facilitate biological family supervised visitation services as requested by DHS / Court.
- Assist and coordinate adoptions for children and families that are eligible.
ONGOING TREATMENT / DISCHARGE
Develop and facilitate each consumers Individualized Service Plan and Service Plan ReviewsAssist foster parents and the foster child with the implementation, tracking, and monitoring of the Individualized Service PlanEstablish relationships of foster parent and foster child through training and supportMeet and / or exceed all state and licensure regulations regarding contact with the foster children on your caseloadEnsure that foster children receive all required medical and therapy services as outlined in the service plan and as required by state regulation.Serve as liaison between OVP and placement agency and all necessary community resource entitiesProvide transportation as neededParticipate in service meetings which may include community support representatives, consumers guardian, biological family, state / county worker, outpatient clinician, foster parents, schoolsystem, and other invested partiesEstablish relationships with school and education personnel to ensure the child is meeting education goals / requirementsProvide ongoing training and support to foster parents and childrenMonitor and review documentation to ensure that it meets all state, licensure and accreditation requirementsMaintain confidentiality as outlined in policyProvide input and / or assist team members in Corrective Action Plan (CAP) for foster parents and execute any assigned tasks related to the CAP.Monitor and review medication logs to ensure foster parents are compliant with medication administration.Ensure that discharge planning includes referral to aftercare services, as applicableComplete home visits as required by rulesCRISIS PREVENTION / MANAGEMENT
Participate in weekly rotating on-call schedule to provide on-call advisory and support to children and foster parents,Document and report any on-call activities and follow-up with leadership.Report to supervisor or to designated authorities any instances ofnon-compliance with OVP, regulatory rules / policies, reports of abuse or suspected abuse, neglect, orsuicidal ideations - immediatelyEnsure all critical incidents are staffed with leadership and entered into the Electronic Health Record within established timeframes.Data EntryComplete and Submit Medicaid documentation as appropriateEnsure complete and accurate data is entered into the Electronic Health Record in accordance with state rules and regulations and company protocol, i.e. ECEM, GeneralContact, Academic supports, medical information, etc.ESSENTIAL DUTIES AND RESPONSIBILITIES :
Transfer established / stable caseload and foster homes to new Case Managers and mentor them through the transitionEnsure that foster parents and case managers needs are met during staff periods of transitionPerform other duties and responsibilities as assigned by immediate supervisorParticipate in the performance quality improvement process and execute any assigned tasks related to the processEnsure that all employee-related documents are current and up to date for your employee personnel fileCOMPETENCIES :

Hold a bachelor's or masters degree in social work or a related human service field, or a bachelor's degree and have completed department-approved training provided by the child placing agencyOne year case management experienceOne year of experience working with children and families preferred21 years of ageValid Drivers LicenseSuccessful completion of all required Criminal Background checks and drug screenings per state and company regulationsWillingness to travel and attend trainingSuccessful completion of all required training pertaining to jobSUPERVISORY RESPONSIBILITY :

This position is community based.Travel is required.Use of personal vehicle is required.Car insurance required.Mileage reimbursed.POSITION TYPE / EXPECTED HOURS OF WORK :
This is a full-time salary position, and core hours and workdays are Monday through Friday with some evening hours. Create your own schedule based on required time in the office and client / foster parent availability. This position may require weekend work.
There is an on-call aspect to this role that will be one week at a time, rotated each week by team members. This is answering referral calls and crisis calls and could require going out to the foster home for placement or crisis management.
WORK ENVIRONMENT :
This job operates in an office and in a community environment. This role may work in an OVP Health Care location periodically, but most of the work will be in foster homes and the community.
