Location : Emergency Room, Hospitals : South Seminole, Dr. Phillips, ORMC, South Lake, or St. Cloud
Schedule : 2 / 3 ten-hour shifts per week with rotating weekends and holidays (Part Time position)
Position Overview :
We are looking for a dedicated and proactive Program Acquisition Coordinator to join our Hospital Care at Home team. As a key player in our patient acquisition efforts, you will be instrumental in identifying referral sources, selecting patients, reviewing referrals, and coordinating admissions.
This role demands excellent communication skills, meticulous attention to detail, and the ability to excel in a fast-paced emergency room and inpatient unit setting.
Key Responsibilities :
Identify Referral Sources : Establish and maintain relationships with referral sources to streamline patient acquisition.
Patient Selection : Evaluate and select patients based on referral information and hospital criteria.
Review Referrals : Assess and review patient referrals to determine eligibility and appropriateness for the Hospital Care at Home unit.
Coordinate Admissions : Manage and coordinate the admission process, ensuring a smooth transition to the Hospital Care at Home unit.
Collaborate with Emergency Room Teams : Work closely with emergency room and inpatient nursing staff and other hospital departments to ensure effective patient flow and care coordination.

Qualifications :
Proven experience in healthcare coordination or a related field.
Strong organizational and communication skills.
Ability to work under pressure and manage multiple tasks simultaneously.
Experience in patient admission processes and referral management preferred.
Flexibility to work four ten-hour shifts per week, including rotating weekends and holidays.
Current licensure as a registered nurse in the State of Florida or Nurse Licensure Compact (NLC) or as a licensed practical nurse in the State of Florida.
One (1) year of experience in the healthcare field.
Associate degree, Nursing Degree from an accredited college or university, or graduate of an approved LPN program.
