Peel Community Care Inc is seeking an experienced Home Care Package Coordinator to join our team based in Pinjarra, WA. This full-time role will be responsible for coordinating and managing the delivery of high-quality home care services to our elderly clients.

As the Home Care Package Coordinator, you will play a pivotal role in ensuring our clients receive the personalised support they need to live independently in their own homes.

What you'll be doing

• Providing Case Management of Home Care packages, including intake, assessments, service agreements, reviews, reassessments, budgeting, care planning and service coordination.
• Liaising with clients, their families, General Practitioners, Allied Health Professionals, community organisations and our care team to ensure seamless service provision.
• Monitoring client progress and adjusting care plans as required.
• Maintaining accurate client records and documentation.
• Providing guidance and support to our care staff.
• Ensuring compliance with all relevant aged care regulations and industry standards.

What we're looking for

• A Diploma in Community Services and / or Diploma of Nursing desirable.
• Minimum 3 years' experience in a similar home care coordination or case management role.
• Excellent communication and interpersonal skills.
• Strong organisational and time management abilities to effectively plan and prioritise work tasks.
• Proven track record in developing and implementing personalised care plans and goal setting.
• Thorough understanding of aged care regulations and best practices.
• Computer literacy, word processing and database competencies.
• A current C class Drivers Licence.
• Current Police Check less than 3 months old.
• First Aid Qualification less than 3 years old.
• The right to work in Australia permanently with no restrictions.
• Empathy, patience, and a genuine commitment to supporting the elderly.
