Overview

We are seeking a compassionate and skilled Admissions RN to join our growing Virtual Care at Home program in Augusta, GA . This unique role combines telehealth innovation with hands-on patient care . You’ll complete in-home assessments, enroll patients into the program, and collaborate with a dynamic care team to ensure patients receive the highest quality support outside the hospital walls.

✨ What You’ll Do

  • In-Home Assessments : Complete patient and family assessments during admission, develop initial care plans, and ensure safe transitions.
  • Patient Enrollment & Education : Introduce patients and caregivers to the Virtual Care program, review treatment plans, and empower families with self-care techniques.
  • Collaboration : Partner with physicians, care coordinators, social workers, and inpatient teams to ensure smooth discharges and continuity of care.
  • Patient Advocacy : Identify barriers to care, address social needs, and work to prevent avoidable readmissions or hospital visits.
  • Innovation & Growth : Actively contribute to performance improvement initiatives, staying ahead of industry best practices.

    • ✅ Qualifications

  • Education : Associate’s Degree or Nursing Diploma required; BSN preferred; MSN strongly preferred.
  • Licensure : Active RN license (Single State or Multi-State Compact).
  • Certifications : BLS required; ACLS preferred within 180 days.
  • Experience :

    • 5+ years of Home Health or Hospice experience preferred.
