  • Clinical Standards : Maintain clinical policies and standards, patient safety, documentation, SOAP notes and medical records.
  • Diagnostics : Order, administer, and record diagnostic tests (e.g., blood tests, X-rays). Diagnose health conditions and illnesses, assess cases presented by your care team, prioritize patient needs.

  • Case Notes : Maintain, update, and escalate accurate case notes, monitor clinical quality gaps in care at the household level, and consult with the care team.
  • Patient Education : Educate patients, families, and non-clinical staff on preventive care and wellness.
  • EHR Utilization : Use Nest Health's electronic health record and care management tools for data collection, documenting member interactions in the field, and communication with your team, members, and community resources.
  • Team Meetings : Participate in daily team huddles and weekly case conferences to discuss health needs and establish health goals in members' Nest Action Plans (NAP).
