Tagalog/Pilipino Speaking Registered Nurse or Exercise Physiologist-Virtual Cardiac Rehab (Remote/Contract/Part-Time)

Background

Movn Health is looking for a Tagalog / Pilipino speaking part time contract care managers to join our team and mission to provide cardiac rehab to 1 million patients every year.

As part of this role, you will be responsible for managing patients on a weekly basis to improve risk factors so they can decrease risk of future cardiovascular events.

This role allows for flexible hours so team members can have varied careers but also have the opportunity to be a part of Movn.

Scope of Work

• Guide participants in 1 : 1 video calls and group education sessions through our 3 month online program to help them manage their cardiac related risk factors.
• Development of comprehensive treatment plans that is tailored to the individual risk factors and medical needs of the patients.
• Treatment plans include exercise prescriptions, medications and interventions for smoking cessation, stress and hypertension management.
• Engage patients weekly to provide coaching, education, emotional support and modify treatment plans through video, voice or text messages.
• Coordinating the approval of treatment plans remotely with Medical Directors.
• Provide progress reports on an as needed basis when symptoms are reported or vital signs are not at goal.
• Share approved treatment plans to referring providers on a monthly basis to provide insight into the patient's performance in the program.
• Monitor dashboard for adverse events and triage appropriately.
• You will leverage our cutting-edge platform to strategically coach participants through the Movn program, using data and analytics to inform outreach methods during moments that matter.
• You will collaborate with our Clinical, Data, Product and Operations teams to discover and drive digital health coaching best practices.
• You will receive comprehensive training and support in the secondary prevention of heart disease by our world renown team including our clinical experts from Stanford Medicine and Kaiser Permanente.
• You will be fully supported by our online platform, which includes a detailed curriculum and technology to help participants track their progress.

Qualifications

• Minimum Bachelor degree in exercise related science.
• Must speak mandarin or cantonese
• Certifications required :
• Exercise related science : ACSM-CEP (preferred CCRP certification or Master's degree or willing to sit for the CEP and / or CCRP exam within 6 months of hire)
• 1-5 years of prior experience in cardiac education, care management, cardiac rehab and secondary prevention required
• Pulmonary rehab and PAD experience preferred as well.
• Optional-Registered Nurse : (preferably with CCRP certification, or willing to sit for the CCRP exam within 6 months of hire)
• Preferably with a compact RN license or willing to obtain a compact license as well as single state licenses in non participating states.
• 1-5 years of prior experience in cardiac education, care management, cardiac rehab and secondary prevention required
• Your work schedule can be flexible (including weekends) across multiple time zones.
• Tech savvy, with a firm grasp on effective and efficient communication through digital platforms such as Slack, Google Meets, Skype, Zoom.
• Experience executing quality improvement initiatives to lower re-admissions and improve patient outcomes.
• A compassionate and empathetic people person who is able and willing to work independently.
