Licensed Practical Nurse (Bilingual English/Spanish LPN)

Position Summary : We are presently seeking dedicated professionals to join Urban Health Plan (UHP) in the capacity of Licensed Practical Nurse.

As an LPN at UHP, you help to shape and transform the health and wellness of families that frequent our outpatient primary care and specialty clinics.

We are currently recruiting a full-time nurse to join our team in the Bronx, NY. As an LPN we will turn to your expertise to be our in-house immunizations expert in our ADULT MEDICINE : -High School Diploma required + LPN Training Program.

• NYS Licensed Practical Nurse Licensure required. -Skilled in phlebotomy and EKGs.-All experience levels are welcome to apply, whether this is your first role as a nurse, or you are coming in as a seasoned nurse.
• Bilingual English / Spanish is required.-Knowledge / training in electronic health records and Microsoft Office. Key Duties : -Performs vitals, comprehensive screenings, and assessments (under supervision of RN).
• Provides vaccinations and blood draws.-Assists with prepping patients as needed and providing patient education. -Assists providers during examinations and treatments.
• Ensure proper sterilization procedures using germicides, sterilizers, or autoclave.-Ensures departmental adherence to clinical policies and procedures.
• Maintains health records by documenting patient contact and updating patient profile. Starting Compensation : Minimum $62,400 / annum;

Maximum $71,000 / annum based on 40 regular hours per , Location, and Schedule : Onsite, Bronx, NY, Monday to Friday 8 : 00am-5 : 00pm / with rotating Saturday shift schedule.
