POSITION : Registered Nurse (RN) (8am - 8pm Tuesday - Friday)
REPORTS TO : DOPS / Registered Nurse
POSTION SUMMARY :
The Registered Nurse (RN) provides nursing care and services to clients in their place of residence / facility in accordance with the agency’s policies and procedures and under the supervision of a Registered Professional Nurse.

QUALIFICATIONS OF THE REGISTERED NURSE (RN) :
- Graduate of an approved RN
- NYS Licensed and currently registered as a Registered Nurse
- MUST have previous Home Health or Long-term Care experience
- MUST hold a current CPR Certification
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
- Ability to perform mathematical operations as related to medication calculations, units of American money and weight measurements, volume, and distance.
- Good organizational writing, communication, and interpersonal skills.
- Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.
- Ability to deal with problems involving several variables in standardized situations.
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

- Clients / clients’ families - Care issues, provide direct client care
- RN / DOPS - Receive direction and supervision
EQUIPMENT OPERATION OF THE REGISTERED NURSE (RN)
Durable medical equipment and nursing equipment.
CLINICAL SERVICES OF THE REGISTERED NURSE (RN) OR LICENSED PRACTICAL NURSE (LPN)
- Follow a designated care plan in accordance with patients needs
- Perform assigned duties. Including administration of medication, wound care, treatments, and procedures
- Monitor clients’ conditions; reporting changes to our RN
- Provides care to clients in accordance with NYS Practice Act
- Implements medical orders and established plans of care
- Follows agency policy and procedures
- Administers medications (oral, IM, SQ, Topical)
- Performs wound care / dressing adhering to sterile technique
- Obtains information from the client regarding complaints, status of illness, problems obtaining services from other sources, problems with supplies and equipment, communication to the DHC as appropriate
- Performs medication review and accountability
- Performs catheter care
- Performs enteral feeding
- Performs ostomy care
- Assists the physician and / or Registered Professional Nurse in performing specialized procedures
- Evaluates the appropriateness, cleanliness, and safety of durable medical equipment, supplies, and or appliances.
- Assists the client in learning appropriate self-care techniques
- May perform aide supervision, as directed
- Documents visits, notes and progress notes to reflect services / client activities
- Communicates with other members of the health care team

PHYSICAL DEMANDS :

Client homes, hospitals, skilled nursing facilities.
