PER DIEM Field Nurse (LVN)
The Doctor’s Choice Home Health Agency* is currently seeking experienced and dedicated *Licensed Vocational Nurses* to join our growing *Home Health Team* covering areas in Thousand Oaks and surrounding Cities.
This skilled nurse will be able to make visits within 10-20 mile distance from their home to the patient’s place of residence, in order to provide skilled care services as ordered by a referring physician.
- Must be available 3 Days / week to pick up cases
- Per-Diem Status* (Nurses choose their own hours and their coverage areas)
- Hours : * You can schedule yourself to see patients between the hours of 8 : 30 AM- 4 : 30 PM.
- Trainings :
- Must be available 1 week day in morning for EMR Training & 1 -3 week days for Field Training in our LA or Orange Office Location.
- Minimum Requirements include : *
- Graduate of a National League for Nursing accredited school of nursing
- Must have minimum 1 year of experience in nursing as an LVN
- Able to see patients at least 3 days a week (REQUIRED)
- Experience in EMR software (preferred but not required).
- Provide skilled nursing services / interventions in accordance with physician orders.
- Assesses patient needs, plans for, evaluates and modifies care to meet goals of skilled nursing interventions.
- Collaborates with all disciplines to plan and evaluate team goals for each patient.
- Experience with Wound Care
- Required Licenses and Certifications : *
- LVN License in California
- Valid Driver’s License & Auto Insurance
- Current CPR certification
- Physical and TB from within the last 6 months
Pay : $43.00 - $50.00 per hour
