Responsibilities :

In coordination with the Performance Improvement Director and Case Manager Director the Discharge RN will be responsible for patient care transition activity from the acute care setting through post discharge according to the transitional plan of care.

The Discharge RN will attend daily bed huddle, round daily on discharged patients, supporting and confirming the discharge plan of care directed by the physician and nurse care giver.

The Discharge RN will screen all discharged patients the following day using the criteria for patients at risk for readmission or non compliance with post discharge care plan.

Each of these patients will be contacted by the Discharge RN for follow up confirmation. The Discharge RN will assume the responsibility for tracking the follow up call results, completing the PHS dashboard, and reporting trends to Case Management and Performance Improvement monthly / quarterly as defined.

Qualifications :

EDUCATION, EXPERIENCE, TRAINING

1. Current valid license as RN in the state of California.

2. BLS on hire and maintain current status.

3. Two years of acute care hospital experience.

4. Spanish speaking is a preferred.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $38.

50 to $55.21. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
