Skilled Nursing Facility experience preferred License : State License required Certifications : COVID test 72 hours prior to start date with results issued 48 hours prior Must-Have : - 8 Hours of Dementia Training- Contactable References (3)- County Criminal Check (7 years)- Federal Criminal Check (7 years)- Multi-State Criminal Search (7 years)- Nationwide Sex Offender Search (7 years)- HHS / OIG Search- GSA Search- Social Security Number Trace- 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray- SAM Search- CORI Massachusetts Background Job Description : Ensure prescreen forms are completed and attached when submitting application.
- Attach resume and license verification; applications with missing items will be declined.- Perform clinical duties and deliver therapy within the specialty of speech therapy.
- Maintain patient records and report on progress.- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Attend and contribute to team meetings and case discussions.- Adhere to professional standards and facility guidelines.
- Show flexibility and adaptability, adjusting to facility and patient needs.- Display strong communication skills with patients, families, and team members.
