Responsibilities :

• Provide primary care services in post-acute care facilities, including skilled nursing and assisted living
• Drive high-quality, safe, and cost-effective patient care
• Collaborate with interdisciplinary teams and follow CMS guidelines
• Work closely with hospice, palliative care, and home care agencies for safe patient transitions
• Provide oversight, guidance, and mentorship to clinicians within facilities

Opportunity Overview :

• Schedule : Monday through Friday
• Patient Volume : 15-20 patients per day
• Compensation : our compensation is a productivity-based model (fee for service) that offers you control to maximize your income potential;

expected compensation is estimated salary range of $171,000 to $277,000 annually; this full-time role is benefit eligible

Qualifications :

• Preferred physicians with a background in internal medicine (IM) or family medicine (FM)
• Physicians with other background who have a passion to get into post-acute care are welcome to apply
