About The Position
Job Title : Physical Therapist Job Specialty : Rehabilitation Therapy Job Duration : 13 Weeks Shift : Monday-Friday, Days Guaranteed Hours : 32 Experience : New Graduate is welcome;
SNF experience preferred License : State License Verification required Certifications : None specified Must-Have : Covid test 72 hours prior to start date with results issued 48 hours prior to start date- Prescreen and resume with license verification must be attached Job Description : Ensure all required prescreening documents are completed and attached upon submitting application- Maintain up-to-date resume and license verification- Conduct therapy sessions in accordance with professional and facility standards- Complete comprehensive assessments and develop individualized care plans- Implement therapy programs to improve client mobility and function- Work collaboratively with a multidisciplinary team- Stay informed of best practices and advancements in rehabilitation therapy- Ensure compliance with all safety and health regulations- Conduct progress assessments and adjust treatment plans as necessary Credentialing Requirements : Resume and three professional references- County Criminal check for all counties lived and worked over the past seven years- Federal Criminal check for the past seven years or more- Multi-State Criminal Search for the past seven years or more- Nationwide Sex Offender Search for the past seven years or more- HHS / OIG Search- GSA Search- Social Security Number Trace- 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray- SAM Search- Negative Drug Screen Results- State Background Check
