In this role you will serve onan interdisciplinary team (IDT) and provide direct medical care toPACE participants. You will work collaboratively with the IDT tocreate and drive forward a care plan that improves health outcomesfor your panel of participants.
In this role you will meetparticipants where they are whether that be in the center clinichome virtually or an external site ofcare.
Requirements
- Mastersdegree orhigher
- NursePractitioner (4years)
- 4years of experience in ANY of the following : Geriatric CareGeriatric / LTC
- ValidAmerican Heart Association Basic Life Support (AHABLS)
- ValidNurse Practitioner (NP) license in the state of California(CA)
