Licensed Practical Nurse - LPN

$37 - $45 Per hour (DOE)

$5 / hr Weekend Shift Differential

PRN / On Call Available

JOB SUMMARY :

A Licensed Practical Nurse (LPN) working in our Skilled Nursing facility is responsible for the daily nursing care needs of our residents.

This position is a valuable clinical team member contributing to the smooth, organized and efficient operations of the nursing department.

RESPONSIBILITIES :

• Provide primary nursing care to residents with varying backgrounds and medical histories
• Assists with new admissions
• A Licensed Practical Nurse (LPN) will monitor and communicate changes of condition to provider
• Provide medical intervention as needed for emergent needs
• Communicate needs of resident to outside providers including but not limited to : Primary Care Provider, Home Health Agency, Pharmacy, etc
• As a Licensed Practical Nurse (LPN) you would communicate resident condition to family, and or responsible party members
• Implement and follow Primary Care Providers orders
• Other Duties as Assigned

REQUIREMENTS :

Licensed Practical Nurse (LPN) Licensure in the State or practice
