sm)*? *Position Highlights : Physician-led, patient centered team-based environment.??

• Providing full spectrum Family / Internal Medicine & Adult Primary Care, focusing on value-based care, and supported by a robust team of specialists.
• Full-time ; Work-Life Balance; Flexible Scheduling (potential for 8, 10, or possibly even 12-hour shifts)!* o *Local office hours are : * o Monday Friday : 8 : 00am 8 : 00pm;

and o Saturday : 8 : 00am 4 : 00pm. o *Full-Time schedules are highly flexible / adaptable, but generally require an equal combination of day & evening shift hour coverage (i.

e., at least 2 shifts per week working until 8 : 00pm, Monday Friday) as well as 2 Saturday shifts per month. 8 or 10-hr shifts are highly preferred!* *Note* : Schedules are confirmed at least 1 month in advance, an ideal candidate would be someone with scheduling flexibility & ability to work combination of day and evening shift hours weekly.

Average patient load* : Goal / Target is 3-4 patients per hour.

There are always 2-3 clinicians working each day of the week / weekend.*

No on-call & inpatient / hospital rounding*. *Primary Responsibilities : *

• Direct and manage a complete continuum of high-quality care within the care team.??
• Treat chronic diseases and disorders.??
• Reviews patient file / record, including allergies, problems, medications, and immunization status.??
• Elicits and records information about patient’s medical history.??
• Orders or executes various tests, analyses, and diagnostic images to provide information on patient’s condition.??
• Analyzes reports and findings of tests and examination and diagnoses condition of patient.??
• Oversees and provides collaborative support to Advanced Practice Clinicians. *Compensation & Benefits Highlights : *

Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
