Job Summary :
Responsible forproviding skilled nursing care to residents according toestablished care plan and in compliance with federal and stateregulations.
Provides guidance and direction to ancillarystaff.
Job Responsibilities andRequirements :
PRIMARYRESPONSIBILITIES
- Provides quality care to acute,subacute, and chronically ill residents, administers medicationsand treatments according to standards of care, and maintainscompetency in all required nursingskills.
- Provides direction to CNAs and monitorsthe workflow of the unit to ensure coordination of care. Usesnursing process for : assessment, nursing diagnosis, planning,intervention, and evaluation.
Report significant changes tophysician as indicated.
Maintains accurateinventory of medications at the beginning and end of each shift.Ensures accurate distribution of medications to all residentsaccording to the 5 rights.
Monitors for medication side effects.Ensures that medications are provided in a manner consistent withmanufacturer's guidelines.
Gives guidance anddirection to auxiliary staff. Participates in report meetings atthe beginning and end of each shift. Communicates with residents,guardians, and family members on the current condition and needs ofresidents in accordance with regulations and standards of practice.
Provides teaching and training to residents and families asneeded.
Ensures that preventative measures arein place such as : specialty mattresses, foam boots, weekly skinchecks, turning / repositioning schedules, toileting schedules, andcushions in all wheelchairs.
Follows up on reports of rashes, openareas, edema, blisters, skin tears, orbleeding.
- Responds promptly to alarms. Ensuresthat restraint alternatives are in place / activated including : personal alarms, cushion alarms, wheelchair belts, tumble mats,alarm mats, bolstered beds, hipprotectors.
- Provides and ensure quality end oflife care. Uses comfort basket to meet resident and family needs.Ensures hygiene, comfort, and spiritual needs are met.
Treatspain / restlessness. Offers family courtesy meal trays and otheramenities. Demonstrates compliance with post-mortem careprocedures.
Participates in departureceremony.
- Works in a constant state ofalertness and safe manner.
- May perform point ofcare testing according to policies andprocedures.
- Performs other duties asassigned.

Graduate of an accredited school ofnursing

- Constant use of speech to shareinformation through oralcommunication.
- Constant standing andwalking.
- Frequent lifting / carrying andpushing / pulling objects weighing 0-25lbs.
- Frequent sitting, reaching and keyboarduse / data entry.
- Frequent use of vision anddepth perception for distances near (20 inches or less) and far (20feet or more) and to identify and distinguishcolors.
- Frequent use of smell todetect / recognize odors.
- Frequent use of hearingto receive oral communication, distinguish body sounds and / or hearalarms, malfunctioning machinery,etc.
- Occasional lifting / carrying andpushing / pulling objects weighing 25-50lbs.
- Occasional lifting / moving ofpatients.
- Occasional bending, stooping,kneeling, squatting, twisting, gripping and repetitive foot / leg andhand / arm movements.
- Occasionaldriving.
- Rare crawling andrunning.
