We are looking for a shift care Licensed Vocational Nurse (LVN) to care for a 61 year old with brain injury, Total care - Trach and G-Tube feeding.
Shifts to be covered :
Every other Saturday 7am - 7pm(12hr shift)
Thursdays or Sundays 7pm - 7am(NOC 12hr shift)

Responsibilities and Duties :
- Assess patients condition during shift and chart observations
- Perform evaluation tasks, including vital signs
- Administer medications / treatments as ordered
- G-Tube care
- Trach Care
- Provide appropriate timely feedback to RN Case Manager and other professionals
Qualifications and Skills :
- CA Vocational Nurse license required
- 1+ yr experience in Home Health
- Recent TB Skin Test or Chest X-Ray
- Physical examination within 6 months
- Health Care Provider CPR Certification
- Comfortable with Feeding Tube / Trach / Vent (agency willing to train)
- Ability to travel (locally) with reliable transportation
