SALARY RANGE : $62,000-$75,000 for LCSW, $73,000-$90,000 for PSY.D

SUPERVISOR : Behavioral Health Director

TRAVEL : Less than 10%

STATUS : Exempt, Full Time

Job Summary

We understand that a person’s behavioral and emotional health is fundamentally important to overall wellbeing, and is closely connected to physical health outcomes.

Our behavioral health professionals are a valued part of the care team, working closely with our medical providers to serve our patients in a collaborative and holistic way.

Behavioral Health Consultants provide targeted individual and group interventions to our patients and community members of all ages, including assessment, brief clinical interventions or short-term therapy to address patients’ goals and improve functioning.

This innovative model of care is designed to reduce barriers and maximize access to high quality behavioral health care for those in our community.

This position has no on-call and no after hours or weekends required

What You Will Be Doing :

  • Provide culturally sensitive, strengths based, trauma-informed care to a diverse population
  • Meet with patients individually in the context of their medical visit to provide targeted interventions addressing behavioral health issues such as depression, anxiety, grief, trauma, developmental concerns, and substance use
  • Collaborate with patients and medical providers to provide effective treatment planning and targeted interventions to help patients set and meet goals for improved physical and psychosocial functioning
  • Create, plan, and facilitate groups in a variety of innovative formats to promote emotional wholeness and address specific patient needs
  • Provide crisis intervention, crisis management, and safety planning as appropriate
  • Document patient progress and services provided in the Electronic Medical Record.
  • Assist with referrals to higher levels of behavioral health care and provide information about other community resources as appropriate

You May Be a Good Fit If You :

  • Are independently licensed as an LCSW, Psy D. LPC, or LMFT in Pennsylvania. Applicants who are seeking licensure are eligible to apply for our Behavioral Health Consultant position.

To carry out its mission, it is of critical importance for Esperanza Health Center (EHC) to keep our patients, families, and workforce safe and healthy and to support the health of our global community.

In keeping with this, EHC has mandated all workforce members on-site at any EHC location for any portion of their time be vaccinated for COVID-19.

This also includes contractors and EHC volunteers. This vaccine mandate is in alignment with applicable local, state, and federal mandates.
