Case Manager Assistant

Job Title : Case Manager AssistantJob DescriptionJoin our dedicated care coordination team as a Case Manager Assistant where you will be pivotal in executing transition of care plans.

You will collaborate with Case Managers and Social Workers in assessing patient needs post-hospitalization, developing and communicating the transition of care plan, and ensuring smooth discharge processes.

This role involves monitoring the progress of care goals, coordinating post-discharge referrals, authorizations, and ensuring all involved parties including the patient and medical facilities are informed throughout the hospital stay.

Your contributions will directly impact patient outcomes by facilitating timely transfers to post-acute facilities and addressing any barriers to the transition of care plan.
