What you will do in this role :
- Assumes responsibility and accountability for the care of those residents treated by Physical Therapist Assistants.
- Assesses resident’s needs; identifies and initiates appropriate physical therapy interventions; collaborates cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
- Observes and evaluates treatment affect, accomplishment of short- and long-term goals. Recommends change to physician, if needed
- Performs an appropriate evaluation on each patient and performs reassessments, as per policy. Performs accurate and timely GG scoring.
The population is adult and geriatric residents in an SNF setting.
Able to assess resident pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain;
makes appropriate physician contact for intervention.
- Formulates and updates resident specific plan of care for physical therapy.
- Other duties as assigned.
Qualifications
- Current Florida licensure as a Physical Therapist
- Must have long term care experience. Knowledge of PDPM, Section GG and LTC program development.
- Familiar with Net Health and Point Click Care
Salary range : $40.00 to 46.00 per hour. We would expect those hired for the job to have a salary within his hourly rate, however, certain circumstances may allow us to pay outside of this hourly rate.
