As a HomeHealth RN Case Manager , youwill :
- Provide admission, casemanagement, and follow-up skilled nursing visits for home healthpatients.
- Administer on-going care and casemanagement for each patient, provide necessary follow-up asdirected by the Clinical Manager.
- Confer withphysician in developing the initial plan of treatment based onphysician's orders and initial patient assessment.
Provide hands-oncare, management and evaluation of the care plan and teaching ofthe patient in accordance with physician orders, under ClinicalManager's supervision.
Revise plan in consultation with physicianbased on ongoing assessments and as required bypolicy / regulation.
- Coordinate appropriate care,encompassing various healthcare personnel (such as PhysicalTherapists, Occupational Therapists, Home Health Aides and externalproviders).
- Report patientcare / condition / progress to patient's physician and Clinical Manageron a continuous basis.
- Implement patient careplan in conjunction with patient and family to assist them inachieving optimal resolution of needs / problems.
- Coordinate / oversee / supervise the work of HomeHealth Aides, Certified Home Health Aides and Personal Care Workersand provides written personal care instructions / care plan thatreflect current plan of care.
Monitor the appropriate completion ofdocumentation by home health aides / personal care workers as part ofthe supervisory / leadershipresponsibility.
- Discharge patients afterconsultation with the physician and Clinical Manager, preparing andcompleting needed clinicaldocumentation.
- Prepare appropriate medicaldocumentation on all patients, including any case conferences,patient contacts, medication order changes, re-certifications,progress updates, and care plan changes.
Prepare visit / shiftreports, updates / summarizes patient records, and confers with otherhealth care disciplines in providing optimum patientcare.

RequiredExperience / Skills :
- Diploma,Associate, or Bachelor Degree in Nursing
- Aminimum of one year of nursing experiencepreferred
- Strong med surg, ICU, ER, acuteexperience
- Home Health experience is aplus
- Current and unrestricted Registered Nurselicensure
- Current CPRcertification
- Strong organizational andcommunication skills
- A valid driver's license,auto insurance, and reliable transportation arerequired.

The compensation range below reflects agood faith estimate of starting base pay for full time (40 hoursper week) employment at the time of posting.
The pay range may behigher or lower based on geographic location and individual paywill vary based on demonstrated job related skills, knowledge,experience, education, certifications, etc.
