- Provide care to patients of all ages and coordinate care with case managers
- English / Spanish bilingual skills a plus
- Monday through Friday, 8 am to 5 pm schedule, with one late day per week
- Position eligible for sign-on bonus and loan repayment assistance
- Hospital-employed position
- Patient census is 22 - 25 per day
- Full insurance benefits included
