7. Drug Screen Lab Report (No longer than 1 year)
8. PPD or QuantiFERON test - (No longer than 1 year) if positive, please bring a proof of chest x-ray that dates within the last 10 years.
9. Proof of MMR / Rubella Rubeola Immunization Lab report
