As a Home Health Physical Therapist , youwill :
- Plan and administer prescribedskilled physical therapy treatment and training for patientssuffering from various injuries, illnesses and functionaldisabilities to attain highest level of physical function.
- Test / screen the patient's physical strengthsto assist the physician in evaluating the patient's level offunction and records findings to develop or pursue treatmentprograms and establish measurable trainingobjectives.
- Develop / implement aconditioning / rehabilitation program consistent with physician'sPlan of Treatment and the overall goals of the patient / rehab team.
Adjust treatment as needed to achieve maximumresults.
- Confer with physician and clinicalteam members to obtain additional patient information and assist indeveloping, implementing and revising the therapy treatment programand Plan of Treatment.
- Provide Physical TherapyAssistants and Home Health Aide staff with writteninstructions / care plan that reflects current plan of care asrelated to therapy, supervise / evaluate staffs' performance.
Monitorthe appropriate completion of documentation by physical therapyassistants and home health aides / personal care workers as part ofthe supervisory / leadershipresponsibility.
- Accurately, promptly andthoroughly document patients' care observations, interventions andevaluations. Assure that interim (verbal) orders received from thephysician and physical therapist are promptly and accuratelydocumented, submitted for physician signature andimplemented
- Report patient's progress to thepatient's physician, Clinical Manager, staff, patient and family.Submit evaluation, treatment plans, progress reports and dischargesummary to the supervisor and care management staff.

- Degreefrom an accredited Physical Therapy Program (approved by theAPTA)
- Minimum of one year physical therapyexperience preferred
- Current and unrestrictedPhysical Therapy license
- Current CPRcertification
- Strong organizational andcommunication skills

The compensation range below reflects agood faith estimate of starting base pay for full time (40 hoursper week) employment at the time of posting.
The pay range may behigher or lower based on geographic location and individual paywill vary based on demonstrated job related skills, knowledge,experience, education, certifications, etc.
