Leverage your therapy skills to assess patient needs, follow care plans, and administer treatments. Collaborate with physicians and community health professionals to deliver comprehensive care.
Responsibilities : Directs and provides physical therapy treatment in the home and assessing and evaluating home environment.
html - Determining priority needs for physical therapy. html - Reporting to physician patient's reaction to treatment or changes in condition.
html - Identifies patient and family / caregiver needs for other home health services and refers as necessary. html - Participates in discharge planning for patient.
html - Supervises physical therapy assistants according to organization policy and state regulations.Benefits : $80,000.
