Physical Therapy Professional (Part-time to Full-time)

Location : Carson, CA

Seeking a Physical Therapist to join our team!!

Physical therapists assist patients with injuries and chronic health conditions in recovering their range of motion, managing pain, and enhancing their quality of life.

PTs employ various techniques, including hands-on therapy, strengthening and stretching exercises, electrical stimulation, ultrasound, ice / heat, and more.

The Physical Therapy Professional assesses and treats residents, as instructed by a physician, using a range of therapeutic methods to restore muscle strength, coordination, and body alignment, alleviate pain, and improve movement and function.

They also collaborate as a member of the interdisciplinary team to provide support and information within the realm of Physical Therapy.

Responsibilities

  • Review resident information and evaluate its impact on the assessment and treatment process.
  • Conduct and document a comprehensive functional assessment of the resident, utilizing standardized procedures. Evaluate home / vocational and equipment needs.
  • Create and implement an individualized treatment plan based on the resident's needs, including achievable functional goals.
  • Adhere to evaluation, treatment, and documentation guidelines, consistently maintaining accurate and timely department records.
  • Instruct Assistants in treatment procedures and provide close supervision while they work with residents.
  • Enter a progress note in the resident record after each treatment session.
  • Modify treatment plans as necessary throughout the treatment process.
  • Create a clear and concise discharge summary documenting the course of treatment according to a prescribed format.
  • Orient residents to program services and appropriate treatment procedures, informing them of any potential risks involved.
  • Collaborate with nursing staff, residents, and families to maximize the resident's functional abilities.
  • Teach residents, family members, or other caregivers about exercise programs and therapeutic procedures to be continued at home after treatment.
  • Assist with the referral process for residents requiring additional services post-discharge, while maintaining knowledge of community resources.
  • Attend and actively participate in rehabilitation team conferences discussing resident progress, issues, or needs.
  • Attend other department / facility meetings as required.
  • Pursue appropriate continuing education opportunities to maintain clinical competency.
  • Participate in the quality improvement process.
  • Promote the programs and services of Heritage Healthcare through interactions with the community.
  • Adhere to regulatory and professional standards and guidelines set by Heritage Healthcare.
  • Provide input to the Rehab Manager regarding the clinical performance of assistants, when requested and appropriate.
  • Follow facility policies and procedures, and actively participate in facility quality improvement and safety programs.
  • Perform other duties as assigned to ensure quality care for residents.
  • Understand reimbursement processes by various payers and effectively allocate therapy resources based on resident goals and discharge outcomes.
  • Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines.
  • Support the company's commitment to Compassionate Care, projecting a professional image that fosters confidence in the Rehab Program.
  • Work independently but recognize situations requiring direct supervision, and willingly assist others.
  • Maintain the expected productivity level discussed and required at the specific facility / facilities.

Qualifications

  • Bachelor / Masters of Science degree in Physical Therapy.
  • Current license in the state of practice.
  • Prefer supervision, training, and / or experience in a setting serving the same age / type of resident population as the facility.
  • Certification, if applicable, and demonstrated competency in specialized areas.
  • Excellent oral and written communication and interpersonal skills.
  • Demonstrated competency in applying the principles, methods, materials, and equipment used in physical therapy.
  • Ability to assess resident needs and develop and implement a comprehensive plan of care.
  • Knowledge of accreditation standards and compliance requirements.
  • Ability to perform various physical activities safely, including standing for prolonged periods, pushing objects, lifting weights, stooping, reaching, balancing, etc.
