Salary Range : $65,000- $80,000 (Pay information will vary depending upon relevant experience for the position) plus Retention Bonus!!

RN Program Coordinator / Care Manager
The RN coordinates all clinical and some administrative components of the Jewish Senior Life Physicians House Call program.
The RN program coordinator triages and helps address patient / family concerns, arranges needed testing in the home, interfaces with home care agencies, communicates with and organizes data for physicians, specialist referrals, scheduling, diagnosis and medical teaching with patients families.
QUALIFICATIONS / SPECIFICATIONS / EDUCATION : The position requires strong organizational and excellent communication skills, mature clinical nursing and / or home care judgment, flexibility, interest in and sensitivity toward frail elders, those with chronic disease, and families dealing with often challenging health concerns.
Ability to work collegially in a team will be essential. Strong computer skills are a must. Registered Nurse with current NYS Licensure.
ESSENTIAL FUNCTIONS :
Daily :
- Triage overnight voicemails.
- Review lab values and imaging results and assign to proper provider
- Review fax inbox and attach documents to proper patients chart if applicable and print faxes for signature and place in appropriate providers bin.
- Review Telephone Encounters and f / u as indicated
- Review progress notes and implement orders, referrals, follow up calls.
- Answer calls as they come in for triage. Return voicemails promptly (15-30 minutes).
- Create TE for each and every call (no matter how trivial they may seem) in eClinicalworks
- Set up referrals for home care as ordered by MD or APP. If a patient is already connected with an agency, use that agency.
Prepare TE for medication refills as they come in and send to appropriate provider for ePrescribe.
- PMP checks for providers opioid prescriptions
- Order home studies for imaging as ordered by Provider, through UltraMobile Imaging.
- Screen new patients
- CCM Care plans and logs
- Interface with Assisted Living communities and pharmacies New med orders, treatment orders, changed orders, DME, 3122 forms.
- Insurance prior authorizations
- Coordinate INR labs, dose changes with patient and caregivers
- Schedule consults, imaging and other testing
OTHER FUNCTIONS
Administrative Responsibilities :
- Works closely with all members of the health care team to ensure high quality coordinated care for all patients.
- Works particularly closely with the Physician House Calls physician(s)
- Team Meeting Agenda weekly
- Populate new patient charts and obtain previous medical records, imaging and labs
- Coordinate immunizations and have vaccine and supplies ready as needed.
- Arrange O2 and equipment with DME vendors
- Run reports to make sure all patients have follow up appointments
- Assists in special projects as directed by the PHC Medical Director or VP of Medical Services.
- Oversee daily tasks / work of Medical Secretary Cover this position when needed.
- Other job duties as assigned.

EXPERIENCE :
RELATION TOTHER POSITIONS : Daily contact with and ability to interact courteously and tactfully with staff, residents, family members, community healthcare agencies and visitors.
PHYSICAL AND MENTAL DEMANDS : Sits 50-75% of the day. May be required to lift (and / or move) 10-30 pounds (most frequently lifts 10-15 pounds).
Occasional walking, stooping, squatting and bending. Repetitive hand / wrist movement. Subject to frequent interruptions.
Physical Work Intensity level 2.
WORK ENVIRONMENT : Works in well-lighted, ventilated office area.
ADVERSE WORKING CONDITIONS : May be exposed to infections, disease and disagreeable odors.
May be subject to hostile and emotionally upset residents, family members and visitors.
VARIABLES : Generally works Monday thru Friday.
