Job Overview :
The Licensed Practical Nurse has the responsibility for providing direct and indirect nursing care to assigned participants.
Works under the direct and indirect supervision of the Jordan Valley Senior Care Home Care Coordinator or designee but is independent in the application of advanced nursing knowledge and skills.
Manages complex clinical situations for which he / she is responsible. Provides nursing care in the PACE center and in the participant's home as may be needed.
Focus of this specific position is wound care for participants with wound care orders due to skin integrity issues.
Duties :
Assessment : Reviews participant's overt problems / needs and assists in setting priorities. Identifies problems requiring further referral and / or follow-up;
observes and records latest diagnostic results; performs advanced nursing observations using critical thinking skills.
Planning Care : Assist in developing and following a plan of care based on nursing process and which incorporates the plans of other disciplines and continuing care needs;
includes the participant / family in developing or revising the plan of care; and makes referrals to interdisciplinary support services.
Initial and periodic assessments (minimally every 6 months) to be completed prior to the scheduled team meetings. Communicates participant changes to center manager / team members.
- Implementing / Managing Care : Be present whenever medical services are provided; ensure care being provided conforms to accepted nursing practice standards and organizational / department standards;
- provide correct treatments / procedures and other care as prescribed and according to participant care standards. Demonstrate thorough understanding of age-related characteristics, symptoms, and needs of participants served;
- explain nursing / clinical procedures in appropriate way; provide participant and family education in coordination of 24-hour care delivery;
identify emergency situations and independently initiate advanced therapy; act as primary coordinator of participant care with other health care personnel and evaluate participant care measures instituted.
Work with primary care providers to furnish the preventive, diagnosis, and therapeutic services prescribed for participants accepted for care by the clinic and to implement physician orders as necessary.
Document all nursing treatment accurately and within departmental time requirements into participants' charts. Understand and demonstrate respect for participant rights and utilize established mechanisms for management of ethical issues in participant care.
- Evaluation : Record participant care delivered as planned and any variation, with appropriate rationale. Make and record observations, as well as implement nursing measures related to impending or associated problems.
- Interdisciplinary Team : Participate as an active and cooperative member of the Interdisciplinary Team if needed. Attend staff meetings and take part in participant care planning.
- Other projects and duties as assigned.

Requirements
Required Qualifications :
- Graduate of an accredited school of nursing.
- Current LPN license in good standing to practice in the required state.
- Minimum 3 years LPN clinical experience with either have one year of experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from the Jordan Valley Senior Care on working with a frail or elderly population upon hiring.
Relevant experience must be within the last 3 years.
- Minimum 1-year wound care experience.
- Other training as required by the department.
- BLS certified.
