Key Responsibilities :
Patient Evaluation and Treatment :
- Assess patients’ physical capabilities and limitations, developing personalized care plans that enhance mobility and reduce pain.
- Implement individualized treatment plans with therapeutic exercises, manual therapy, and various modalities to improve functional abilities.
Patient Education and Support :
Educate patients and their families on the benefits of physical therapy, providing support throughout the recovery process.
Collaboration :
Partner with healthcare professionals, such as physicians, nurses, and occupational therapists, to foster a collaborative environment that supports patient recovery.
Documentation :
Maintain accurate patient records and ensure effective communication with other healthcare providers, supporting continuity of care.
Professional Development :
Stay current with advances in physical therapy through ongoing education and professional development.

Qualifications :
- Licensing : Must hold a valid Physical Therapist (PT) license in Arizona.
- Experience : Minimum of one year of home health experience.
