Registered Nurse - Hospice
About the Role : Join a leading hospice care provider to make a difference in the lives of patients and their families. As a Registered Nurse - Hospice, you will be responsible for providing compassionate care and support, while collaborating with an interdisciplinary team to ensure the highest quality of care.
Key Responsibilities :
- Report to the Administrator, Executive Director or Patient Care Manager Senior
- Act as the primary liaison between physician, patient / family, and the hospice team
- Provide direct patient care services and supportive care to the patient's family
- Educate and empower patient's caregivers
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
- Maintain patient clinical records
- Participate in company quality assessment and performance improvement programs
Qualifications :
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
- Current driver's license and automobile insurance
- Current CPR certification
