The Speech Language Pathologist Home Care position is PRN.Schedule Expectations : PRN hours will vary based on coverage needs.
Open to locations around the Cleveland area.Evaluates and treats patients, mentors, supervises work performed by staff and contributes to the management of daily operations.
Responsibilities : Performs initial and on-going assessments of patient's condition and establishes goal and plan of care which are appropriate to problems identified and involves the patient / family according to their capabilities and desires.
Performs therapy interventions utilizing standard speech therapy techniques and skills as appropriate for the age and condition of the patient.
Evaluates communication and swallowing function of patients.Selects assistive speech devices to improve patient's communication.
Maintains complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
Participates in ongoing interdisciplinary assessment of assigned patients. Participates as a member of the hospice interdisciplinary team when necessary.
Other duties as assigned.Education : Master's Degree in Speech / Language Pathology.Certifications : Current Speech-Language Pathology licensure (SP) in the State of Ohio.
Holds Certification of Clinical Competence (CCC-SLP) from the American Speech Language Hearing Association (ASHA).Current Basic Life Support (BLS) certification required through the American Heart Association (AHA).
Valid Ohio Driver's license upon hire; per policy 8-175 thereafter.Proof of automobile insurance coverage with $100,000 / $300,000 coverage, upon hire;
per policy 8-175 plexity of Work : Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment.Work Experience : Minimum of one year experience as a Clinical Speech Language Pathologist preferred.
Physical Requirements : Ability to stand for long periods of time.Manual dexterity and visual acuity sufficient to perform tests.
