Standard Hours : Monday - Friday normal daytime business hours (no on-call, no weekends and no holidays required). This is a field based position in the greater Washington DC area and you will be in the field % of the time going into members homes.

Primary Responsibilities :

  • Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care in their home
  • Manage members through an acute medical situation and work with chronic conditions, including testing, diagnostics, interventions
  • Identify, initiate, and follow up on referrals for community based programs and providers; including financial, psychosocial, community and state supportive services
  • Manage the care plan throughout the continuum of care
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications :

  • Associates degree or higher
  • Current unrestricted Registered Nurse license in Washington DC
  • Certified in Basic Life Support
  • 1+ years of experience in post - acute care, such as long-term care
  • 1+ years of experience with using an Electronic Medical Record
  • Valid Driver's License
  • Access to reliable transportation
  • Ability to travel up to % to visit members in their homes within Washington DC

Preferred Qualifications :

  • 1+ years of experience working with the geriatric population
  • Experience conducting inservices
  • Field based experience going into patients homes
  • Experience creating care plans
  • Case Management experience

Washington, D.C. Residents Only : The salary range for this role is $58, to $, annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
