Position Description :
A Wound Care Nurse plays a crucial role in a skilled rehab nursing home by overseeing and coordinating all aspects of wound care for residents.
This position requires a highly skilled and knowledgeable individual who can provide comprehensive wound assessment, treatment, and education to promote optimal wound healing and resident well-being.
A Wound Care Nurse collaborates with the interdisciplinary team, including physicians, nurses, therapists, and other healthcare professionals, to develop and implement effective wound care plans.
Additionally, a Wound Care nurse stays up-to-date with the latest advancements in wound care management and ensures compliance with regulatory standards.
Responsibilities of a Wound Care Nurse (RN) :
- Responsible for overseeing the Wound Care Measures
- Must have up-to-date MMR vaccination & physical
- Responsible for overseeing the infection control program
- Communicate changes in residents' conditions to IDT and other partners as appropriate
- Participate in the on-call rotation with other nursing supervisors
- Flexible working hours
- Attend Quality Measures meeting and report on applicable information
- Attend and prepare for weekly Case Management meetings
- Includes but not limited to items listed on the Registered Nurse Job Description
- Ability to educate partners by conducting in-services and one on one learning
- Ability to investigate, track and trend identified areas of opportunities
- Develop action plans for areas of needed improvement
- Provide partner support and coaching
- Passion for wound care and healing residents

Requirements of a Wound Care Nurse (RN) :
- A current NYS Registered Nurse license required
- Excellent physical assessment and critical thinking skills
- Excellent time management skills
- Ability to perform effectively in a fast-paced environment
- Excellent communication skills
