Promenade Management -
SUMMARY : The registered nurse is responsible for planning, organizing, directing, and coordinating healthcare and professional services of the Licensed Home Care Service Agency and the Assisted Living Program.
This position oversees the day-to-day clinical operation of home care in conformance with the organization’s mission, philosophy and goals.
As the Director of Patient Services they ensure that all aspects of home care / programs operate within the standards of all applicable laws, rules and regulations.
Responsibilities :
Assisted living resident evaluation and admissions
NYS UAS initial evaluation, six months, change in condition and annual review
Development of paraprofessional care plan
Coordination and supervision of all client services
Interaction with physicians to ensure optimal client care
Maintains the confidentiality of all protected health information as set forth by the HIPAA Privacy Rule.
Makes visits to hospitals, rehabs and SNF’s for current / prospective resident evaluations
Ensures that the Medication Administration Record is complete and current
Development of agency policies and procedures
Agency Quality Assurance and compliance
Ensuring documentation and submission of relevant LHCSA statistics
Reporting overall LHCSA performance to Management
Oversees the recruitment, training and supervision of LPN / HHA / PCA positions
Completes a performance review and all subsequent annual reviews
Coordinates first aid and in-service training for wellness staff
Interacts with residents, families, visitors, and staff to properly coordinate resident care needs / requirements
Ensures appropriate coverage based on resident census / requirements in wellness area
Maintains the confidentiality of protected health information as set forth by the HIPAA
Qualifications :
Current NYS Licensed Registered Nurse.
Graduate of an accredited nursing school
Two (2) to four (4) years experience in a patient care setting, acute care desirable.
Two (2) years’ experience in a supervisory or administrative position in a home care organization OR an equivalent and demonstrates a strong knowledge of regulations.
Excellent clinical, writing and communication skills.
Familiar with NYS DOH regulations
May require lifting or bending. Must be able to lift 25-35 pounds
