Key Responsibilities :
- Client Care : Develop individualized care plans meeting Medicare guidelines.
- Team Leadership : Manage and support our skilled nurses, therapists, and aides.
- Compliance : Ensure services meet all Medicare and state regulations.
- Coordination : Work with licensed physicians to provide necessary skilled nursing and therapy services.
- Client Support : Assist with personal care, grooming, transfers, ambulation, and exercises.
What We’re Looking For :
- Licensure : Active RN or LPN license in Minnesota with at least 5 years of experience in home health care.
- Skills : Strong clinical abilities and in-depth knowledge of geriatric healthcare.
- Communication : Excellent interpersonal and collaboration skills.
- Organization : Ability to prioritize tasks and manage time effectively in a fast-paced setting.
- Approach : Compassionate, patient-centered, and proficient in electronic medical records.
